Getting a health card

After registering your lump-sum tax agency, to obtain a health card you will need:

FOR YOU

  • a copy of the MA form from the BRA
  • ID card (copied)
  • a copy of the BRA registration decision and the document containing the tax ID
  • health card
  • With complete documentation, you need to go to the NHIF (National Health Insurance Fund) of the municipality of your headquarters to receive a health card (unless your place of residence is in another city, and in that case you will go to the NHIF of the municipality of your residence).

If you didn't get the MA form in BRA, you need to submit the above-mentioned documentation to the PIO fund (in the city where your agency is registered) and receive the MA from them. After that, you can follow the steps above.

FOR YOUR SPOUSE

  • filled M ​​form
  • ID card of the spouse (copied)
  • marriage certificate
  • a copy of the BRA registration decision and the document containing the tax ID
  • With complete documentation, you need to go to the NHIF (National Health Insurance Fund) of the municipality of your headquarters (unless your spouse’s place of residence is in another city, and in that case you will go to the NHIF of the municipality of her/his residence) to receive a health card for your spouse.

FOR YOUR CHILD

  • filled M ​​form
  • certificate of residence
  • birth certificate
  • a copy of the BRA registration decision and the document containing the tax ID
  • With complete documentation, you need to go to the NHIF (National Health Insurance Fund) of the municipality of your headquarters (unless your child’s place of residence is in another city, and in that case you will go to the NHIF of the municipality of her/his residence) to receive a health card for your child.
